Mineral Trioxide : Material Of Choice for Endodontic Application

Mineral Trioxide Aggregate (MTA) is a relatively new material that has become the material of choice for certain endodontic applications like Pulp Capping, Perforation Repair ,Root end fillings.
A significant problem in restorative dentistry and endodontics is isolation of operative feild for moisture control. The root canal system to be obturated must be dry in order to obtain a good good seal and comunication with blood must be avoided.During the direct pulp capping procedure ,hemorrhage must be controlled.When attempting to seal a root perforation,a dry field is  essential.Further during apical surgery,the retro preparation must be absolutely dry.
Recently , Torabinejad and colleagues developed anew cement named Mineral Trioxide Aggregate(MTA), which appears to have all the characteristics of an ideal cement to seal communication between the root canal system and the oral cavity and between root canal system and the periodontium. MTA is an endodontic cement that is extremely bio compatible,capable of stimulating healing and osteogenesis, and is hydrophilic. In simple terms it has a powder which sets in presence of moisture.The cement is different because of its biocomatability, antibacterial properties sealing properties and hydrophilic nature.
Recently the prognosis of the teeth with a perforation has improved with use of operating microscope and the introduction of MTA.Primarily because the MTA is hydrophilic ,it today can be considered the ideal material to seal perforations.In fact,cementum has been shown to grow over MTA,allowing for normal attachment of the periodontal ligament.Furthermore , MTA doesn’t require a barrier,it is not affected by moisture or blood contamination, and seals better than any other material in use today.
The operating sequence to treat a perforation of root or of the floor of the pulp chamber is completed in two visits.The operating field is isolated and perforation site is cleaned.In case of bacterial infection calcium hydroxide is applied for 1 week otherwise 2-3mm MTA is applied and checked radio graphically for position of the material .A wet cotton pallet is applied and temporarily cemented. After 24 hours in second visit the temporary cement is removed and MTA setting is checked.

Criteria For Assessing Success
To be able to state that success has been achieved following treatment of perforation ,the treated tooth must meet following requirements:
* Absence of symptoms such as spontaneous pain or pain on percussion
* Absence of excessive mobility
* Absence of communication between the perforation and the gingival crevice
* Absence of fistula
* Normal Function
* Absence of radiolucency adjacent to perforation
* Thickness of PDL adjacent to obturating material no more than double the thickness of adjacent ligament.If even one of these criteria is lacking the therapy is not successful.

Tooth Replacement :Dentures

There are many reasons why people lose their teeth including dental caries, periodontitis, and tooth trauma.  Dentists have been using dentures for numerous years to replace lost teeth. There are also other ways to replace lost teeth. Compromised oral function has been linked to decreased self-esteem and a decline in the quality of life. Adults with missing teeth or loose dentures may avoid certain social activities because they are embarrassed to speak, smile, or eat in front of others. Your teeth provide support and structure to your face and cheeks, and lost teeth may result in sagging cheeks, making you appear older than you really are. By replacing your teeth, you can remove years from your face.
Dentures are removable appliances to replace your teeth, and are made to look, feel, and function naturally. They are made to look like your teeth and gums, and look very realistic and natural and have a comfortable feeling. There are several different types of dentures, and these include partial and full dentures. Partial dentures are made when there are some teeth that are in good condition still remaining in your mouth, and full dentures are made when there are no teeth remaining in your mouth. Partial dentures are clasped onto your existing teeth, and will keep those teeth from shifting in your mouth. Full dentures are held in place by suction created on the roof of your mouth when the dentures are in place. Lower dentures do not have the same suction, due to their smaller surface area and your tongue and muscles of the mouth moving. The best way to care for your dentures is to make sure they are kept clean. Your dentures should be removed while you sleep to allow your gums to rest, and allow them to be exposed to your mouth environment in order to keep them healthy. When you remove your dentures, you should brush them and soak them in cold water, or a cleaning tablet specifically made for this purpose that you place in water. DO NOT USE BLEACH! Also, brush your dentures again before replacing them back into your mouth. If your denture has large buildups and debris that you cannot remove by brushing, visit your dentist and have them or their dental hygienist remove the debris and clean your dentures. Even though you don’t have your natural teeth, it is very important for you to visit your dentist on a regular basis, in order to make sure that your dentures and your underlying supporting tissues are in good shape. They will also fit your dentures better if they are getting loose from shrinkage of the underlying tissues. Dentures that do not fit properly, cause gum irritation and inflammation, and other problems such as bad speech and inability to chew food.
Dentures can last quite a while if they are properly maintained by the patient and you visit your dentist regularly. There is some natural wear and wear that occur to dentures, especially those made with plastic teeth. These teeth wear more quickly than porcelain teeth. Also, your gums and underlying bone tissues may shrink causing your dentures to get loose and changing your bite in the process. Your dentist will reline or rebase your denture depending on the situation. However, it is highly recommended that you have new dentures made after five years.
Your dentist will perform a complete examination and take radiographs of your mouth. After discussing your needs and wants, your dentist will determine if dentures are right for you. If you already have an old denture, your dentist may use your old denture to take an impression of your mouth. If not, the dentist will decide if there are some questionable teeth remaining in your mouth that cannot be saved. After this is decided, the dentist will take impressions of your mouth and a bite registration, and stone models will be made of your mouth in the dental laboratory. Dental technician(s) will fabricate the best possible dentures according to your dentist’s instructions.
If you need to have teeth extracted, “immediate” dentures will be fabricated prior to teeth removal, and following the surgery, the denture is inserted immediately. This aids in the healing of the extraction sites and will act as a surgical stent. It allows you to function and have better aesthetics during the healing period. These dentures usually require relining with soft and/or medicated material while the tissues are healing and shrinking. “Regular” full dentures are fabricated after the healing is complete. This denture can usually be inserted 8-12 weeks after the teeth have been extracted.

Contraindications for Implants

Dental implants in India have become a very popular choice for many patients who would like to replace missing teeth with the most natural option possible. Implant dentistry is highly predictable, offers greater flexibility with regards to treatment possibilities, and can give patients fully functional teeth in as short as an hour with minimum discomfort and pain using a local anesthetic with or without sedation.
There are few absolute contraindications to implant dentistry.However there are some systemic, behavioral and anatomic considerations that should be assessed .
Failure to precisely locate the IAN (inferior alveolar nerve )and MF (mental foramen )invites surgical insult by the drills and the implant itself. Uncontrolled type II diabetes is a significant relative contraindication as healing following any type of surgical procedure is delayed due to poor peripheral blood circulation. Implants are contraindicated for some patients who take intravenous bisphosphonates which may put patients at a higher risk of developing a delayed healing syndrome called osteonecrosis. Bruxism (tooth clenching or grinding) is another consideration which may reduce the prognosis for treatment. The forces generated during bruxism are particularly detrimental to implants while bone is healing; micromovements in the implant positioning are associated with increased rates of implant failure.

Modern implantology uses techniques that can provide function, esthetics, and comfort with a minimally invasive surgical approach. Flapless implant surgery has been proposed to fulfill these requirements. Traditionally, flapless implant surgery was carried out by using a tissue punch technique, which may be potentially harmful because of the inherent blindness of the technique.
Using dedicated interactive computer software programs and 3D radiographic techniques such as computed tomography (CT), the precise location of each implant is planned. Using the concept “prosthetic-driven implantology,” surgery is carried out for the rehabilitation of patients. Based on three-dimensional implant planning software for computed tomographic (CT) scan data, customized surgical templates and final dental prostheses are designed to ensure high precision transfer of the implant treatment planning to the operative field and an immediate rigid splinting of the installed implants, respectively. There is no doubt that three-dimensional oral implant planning software, of both surgical templates for flapless surgery and dental prostheses for immediate loading is a very reliable treatment option.
