Immediate Loading Dental Implants

“One of the most critical (perhaps the most critical) factors in successful osseointegration of an implant is stability in the bone at time of placement. Relative motion between the implant body and the surrounding bone during the early healing phase is considered to be a high risk factor for early implant loss as failure of osseointegration occurs” The term osseointergration means how the implant fuses with the bone. After tooth removal, the residual bone that is used to support the teeth will then shrink away quite rapidly over the first year because the body knows that the residual bone is no longer needed. The greatest amount of shrinkage occurs during the first year after tooth removal. Research has shown that wearing dentures will accelerate jawbone shrinkage. As the shrinkage of the jawbone support progresses over time, new dentures will need to be fabricated in approximately six to ten years.Because each denture wearer is unique, sometimes the internal surface of the denture needs to be readapted to the gum tissues. This procedure is called a reline.

Some patients are never able to adapt to functioning day-to-day with their complete dentures. Many of these patients do not have adequate bone to stabilize a lower denture. Dental Implants – Threaded Expanders For Bone Expansion

People lose teeth all the time, either through trauma (when teeth are knocked out) or due to decay, gum disease or old age. Whatever the reason for losing your teeth, they need to be replaced, both for aesthetic and functional reasons. The most common treatments for missing teeth are either a denture or a fixed bridge. However, dental implants are now becoming a more popular treatment to replace missing teeth, as they provide a longer-term solution, slow down bone loss and preserve nearby healthy tooth tissue.

Threaded expanders are another group of instruments for bone expansion. They work by rotation, which differentiate them from the osteotomes, which work by impactation. Threaded expanders have an aggressive conical shape and penetrate the bone easily. The principal advantage is that they are less traumatic to the patient and that they can be used in the lower jaw. The rotation movement can be done with a wheel inserted in the back part of the expander, or if more resistance is present, with a racket wrench.

The principal disadvantage is that is easier to loose the direction of the preparation compared to the osteotomes. Anyway, in both cases an adequate training is important.
